Required Compliance Documents

Every year, our organization is required by state law and the rules of the Buckeye Valley Summer League, to have all players and a parent sign some required documents. For both baseball and softball, you are required to view the materials on Lyndsay’s Law and the Concussion fact Sheet. We will need a signature from at least one parent and the player.We will be able to colect those signature digitally this year.

After viewing the required links, a parent and the player should sign the form and submit it. You can only submit one player per form as we are tracking these by teams. However, the same parent may sign multiple forms.

Failure to sign these documents, will result in the player not being allowed to play in their games.
